Zepbound multi-dose KwikPen to be preferred on formulary starting August 1, 2026

CarelonRx is making changes to the DMHC National and Essential formularies. As of August 1, 2026, the Zepbound multi-dose KwikPen will be preferred for fully insured large group clients on the California DMHC National and Essential formularies. The Zepbound single-dose auto injector will move to non-formulary. There will be no grandfathering for this change.

  • Any new Zepbound user after August 1 must obtain a prior authorization and prescription to align with this formulary change.
  • Members currently using the single-dose auto-injector must transition to the multi-dose KwikPen by October 1.
  • Members with a current prior authorization that ends before October 1 will need a new PA through the standard process when that authorization terms.
  • Current users will be notified of this change via letter on August 1 and September 1.
  • New prescriptions will be needed for the multi-dose KwikPen and pen needles. Prescribers will be notified of this change and will be asked to provide a new prescription.
